B. Dissolution of Marriage, Legal Separations, and actions to declare a marriage invalid, void, or annulled.
C. Post-Dissolution of Marriage matters, including but not limited to modification of custody, visitation, child support, and maintenance.
D. Adoption.
E. Orders of Protection and No Contact Orders.
F. Parentage and support actions.
G. Other family law matters including but not limited to name change(s), grandparent visitation rights, abuse, neglect and dependency matters, guardianships, and prenuptial agreements.
H. Debt collection actions where the amount in dispute is $50,000.00 or less and the Participant and/or a Dependent are not otherwise insured, and the matter does not involve the Life Credit Union.
I. General counseling of five (5) hours per Participant per year; no additional coverage to Dependents.
J. Traffic and Reinstatement of Driver’s License Privileges, although not including DUI or any felony charge.
K. DUI: 100% coverage except no coverage shall exist for any subsequent DUI charge incurred within 36 months of the date of disposition of the prior DUI charge.
L. Misdemeanors (Not otherwise covered hereinabove).
M. Felony: No coverage for any matter charged as a Class X or Class 1 felony; no coverage for any felony charge subsequently incurred within 60 months of a felony conviction covered under the Fund. Any class 2, 3, or 4 felony will be covered, except in any of the following circumstances:
1. A weapon was allegedly used by the participant or any accomplice in the commission of the felony;
2. A minor child was allegedly involved in the commission of the felon;
3. Any alleged conduct constituting a sex offense under the Illinois Criminal Code of 1961;
4. The manufacture, delivery or sale of any controlled substance or a look-alike substance or counterfeit substance (as defined by the Illinois Controlled Substance Act [720 ILCS 570/100 et seq.] in any school, or any conveyance owned, leased, or contracted by a school to transport students to or from school or a school related activity, or residential property owned, operated, and managed by a public housing agency or public park, on the real property comprising any school or residential property owned, operated, and managed by a public housing agency or public park or any public way within 1000 feet of the real property comprising any school or residential property owned, operated, and managed by a public housing agency or public park, except if filed as a juvenile charge or if, in the sole opinion of the Provider, the Participant (or any qualified dependent) is eligible and is reasonably likely to receive first offender probation under 720 ILCS 570/410, now in existence or as may be amended from time to time.
